Consent to walk home from school alone and conduct regarding mobile phones at school (Years 5 & 6 children only)

Please complete the form to indicate your consent for your child to walk home from school on their own and expected conduct when bringing a mobile phone to school. This is only appropriate for Year 5 and 6 children.

I give permission for my child to bring a mobile phone to school. I understand the school takes no responsibility for the phone.

I understand that my child will need to switch their phone off whilst on the school site, give it to their class teacher at the start of the day and collect it at the end of the day.

I understand my child must not use or have their mobile phone on display at school at any time. If a member of staff identifies a mobile phone on display/in use it will be removed and placed in the school safe until the end of the school day.

I will ensure the above is clearly communicated to my child/ren.

I give permission for my child/ren to walk home alone at the end of the school day.

I confirm a responsible adult will be at home to greet my child/ren when they return home from school at the end of the day.

I understand the responsibility for my child's safety on their journey to and from school rests with me.
